Forms submitted to the Department must be printed on a laser or inkjet printer with a resolution of 300 dots per inch or higher. Forms printed on a dot matrix printer cannot be accepted.

The Louisiana Department of Revenue has made the most commonly requested tax forms available in electronic format. Current forms include the current year forms and forms for the previous five years.
These forms have been formatted with a fill-in feature that allows you to type information into them on your computer and then print out a paper copy. In order to use the fill-in feature, you must have the Adobe Acrobat Reader which is available for free online.
The most commonly used Massachusetts tax forms and helpful publications can be found within this area of the Department of Revenue Web site. You may search for a form by category or via the alphanumeric Online Forms Index.

Forms for tax years 2008, 2007, 2006, 2005, 2004, 2003, 2002, 2001, 2000 and 1999 are available to download. You should file the original that you print and keep a photocopy for your records. The original that you print is not considered a photocopy. You should not duplex print these forms.
